Insurance

Please bring a referral from your family physician (if required by your plan) and a copy of your insurance card with you. We bill most insurance carriers for covered services, as long as proof of insurance is provided at the time of service. Co-payments are expected at the time of service. Payment for services is the responsibility of the patient or guardian of the patient. We accept cash, checks, Visa, and MasterCard.

We have contracts with some, but not all, insurance companies. If we do, you will be responsible for any deductibles or co-payments at the time services are rendered. Your insurance policy is a contract between you and your company; we are not a party to that contract unless we also have a contract with your company. If your insurance company has not paid on your claim within 45 days, you will automatically be responsible for the balance.

Please be aware that some, and rarely all, of the services provided to you may be deemed non-covered services or not medically necessary under Medicare and/or other medical insurance programs.

Patients are responsible for obtaining any referrals required by specific insurance companies. Insurance information (insurance card) is required at each visit as well as a photo ID. All co-payments or outstanding balances will be collected prior to being seen.

If a patient has insurance with which we do not participate, payment in full is required. A copy of the charges will be given to the patient who can then file for reimbursement.. Cosmetic Services

Any procedures considered to be cosmetic are not covered by insurance, and therefore payment is expected at the time of service. We will not file any cosmetic service to insurance. We will notify you if we believe a planned procedure is cosmetic and will discuss costs prior to the procedure.
